5 Years Success Story of Happy Eating

Follow ups are the true essence of success.


Here is a case we had done 5 years back in May 2017. She presented with a broken tooth (upper left 2nd premolar) & a missing tooth (upper left 1st Molar). We got the CBCT Scan done. With little amount, it was a challenging case. The patient did not want to undergo any invasive procedures like sinus lifts.


Such a situation can be managed easily with cortico-basal implants. They can be placed immediately after tooth root removal & even through the sinus (trans-sinus). This is possible because of the polished surface of the implants which are resistant to any infection. The apical threads of the implants engage the basal bone which is a dense cortical providing rigidity & stability to the entire framework.


After extraction of the tooth (25), we placed 3 Cortico-Basal Implants, 2 engaging the palatal basal bone. It was difficult to bypass the root of 24, hence in the extraction socket of 25, we placed BCS-EX implant, it corticalises the cancellous bone & provides stability.




A Ceramic prosthesis was placed on the 3rd day & luted with Type 1 GIC. Occlusion was checked & an implant protected occlusal scheme established.



We had a couple of follow up appointments, later after she saw us in December 2022 after 5 years 7 months. She is comfortable, happily eating & enjoying her life. We did a clinical examination; she has maintained her teeth immaculately. Very rarely do we see people with such excellent hygiene care. Implant supported teeth are great, stable and she has a crown on tooth 27.



We took an OPG X ray & all looked great. There were a couple of high points which had developed on the implant teeth. They were taken care of, and she was scheduled for her next follow up at 6 months.




On closer examination, we can notice a considerable recession of tissues around the implant teeth on the buccal side. However, the soft tissue is in its pink of health. Why did this happen? I can hypothesize a few reasons

1. When we started, we had less bone in the region

2. As age advances there is sinus expansion

3. High point on the implant crown

Can you think of any other reasons?


Is it a cause for worry? Absolutely not.

1. The region is in the non-aesthetic zone,

2. There is no problem in keeping the area clean,

3. The gingiva is healthy.


This is one of the advantages of Basal implants. Because the zone of anchorage is away from the alveolar crest, such bone loss does not affect the stability of the implants. If it had been a conventional rough surface implant, it would have landed up in peri-implantitis with such bone loss.


Follow ups are the true essence of success. - To understand the success of our work, we must follow up with our patients regularly, these follow ups teach us a lot.
